How to Transfer Saves from PS4 to PS5 for Next-Gen Upgrade in The Witcher 3

The Witcher 3 save can be moved from a PS4 to a PS5 for the next-generation edition update and upgrade.

The following steps have been provided by Reddit user lordmorpheus2000:

First open the ps5 version of The Witcher 3. Go to load menu and then press L2 to open the gog or code. Scan it with your phone and create a gog account or log in you already have one.
⁠You’ll receive some awards, accept them and then press L2 again and the game will tell you cross progression is enabled.
⁠Download the PS4 version of The Witcher 3 if it isn’t already installed on your console.
⁠Open the PS4 version and load the save from where you want to start the game on PS5 (you will find your latest ps5 version save here, ignore that)
⁠Pause and save the game again(this is the save that will transfer) and then go to the load menu again. And then press L2 trigger to open the GOG QR code like before.
⁠Scan it with your phone and then login with the gog account.
⁠You’ll again receive some awards and then again press L2 and the game will tell you that your save has been uploaded to the cloud.
⁠Close the Ps4 version. Start the ps5 version again and then go to the load menu and you’ll find that ps4 save you made there. Enjoy!

In addition to everything mentioned previously, a number of Reddit users also pointed out that the PS4 version needs to be updated to version 4.0. You cannot move your save to a PS5 if your PS4 save file is version 1.6.
